HARTZ Search invites you to explore an exciting opportunity with our client Carolina Collaborative Community Care (4C), located in Fayetteville, NC, for the Executive Director role. The Executive Director will report to the Board of Directors and has administrative responsibility for the operations and management of Carolina Collaborative Community Care (4C) which includes clinical, financial, and human resources. Carolina Collaborative Community Care is an NCQA accredited organization incorporated in 2004 that offers care management and disease management services in Cumberland and surrounding counties. 4C maintains a focus on population management, recognizing and addressing medical and non-medical social drivers of health and well-being to ultimately improve the quality of patients’ lives.


The Director is accountable for ensuring mechanisms are in place for the 4C team to achieve effective and measurable clinical, financial and operational outcomes. Other responsibilities include but are not limited to ensuring the collection of service-related data and evaluation of processes and outcomes to facilitate quality improvement. The Director will also promote a safe work environment that ensures staff accountability for confidentiality, Health Insurance Portability and Accountability Act (HIPAA) compliance, federal and state workplace rules and regulations, and compliance with 4C policies and procedures.

  • Ensures that 4C has a long-range strategy to achieve its corporate mission, goals, and objectives
  • Provide leadership in developing programs and organizational plans for 4C in collaboration with all 4C contracted partners and the 4C Medical Director
  • Maintains and utilizes working knowledge of significant developments and trends in the area of best practice for all contracted 4C programs
  • Demonstrates quality of analysis and judgment in program planning, implementation, and evaluations
  • Promotes effective communication with physicians, nurses, and other members of the interdisciplinary care team to effect timely and appropriate patient management.
  • Functions as a role model and ensures that clinical leadership staff and case management staff work collaboratively and maintain active communication with physicians, nursing, and other members of the interdisciplinary care team to effect timely and appropriate patient management
  • Collaborates with the care management and clinical leadership teams with their efforts to assist patients with resource management needs
  • Communicates and collaborates with the interdisciplinary team
  • Reviews utilization management and quality screening patient data, as needed
  • Leads development team and oversees the framework for 4C’s strategic plan
  • Coordinates and oversees committee structure to include appropriate members of organizations and the community
  • Actively participates in the initiation, development, and maintenance of population-based disease management programs, facilitating collaboration and consensus of interdisciplinary teams throughout the process



  • Bachelor’s degree in health or business required
  • Master’s in health-related field preferred
  • Licensure in the State of North Carolina as appropriate
  • Excellent interpersonal, communication, and negotiation skills
  • Strong organizational and time management skills
  • Understand principles and operations of patient medical and mental care and care management supports
  • Understand the principles of quality improvement in the health care setting
  • Ability to work with a variety of health care professionals in the hospital, academic, and community settings
  • Experienced in facilitation and group process
  • Strong administrative experience in the area of employee supervision and financial accountability
  • Ability to prioritize disease and care management activities
  • An understanding of the Medicaid population, Medicare population, and general patient care continuum
  • Knowledge of the best processes and materials to provide education and outreach services
  • Understand the importance of evidence-based clinical guidelines in quality improvement
  • Understand population management and stratification and prioritization of care management activities



The City of Fayetteville is a thriving community located in the Sandhills region of southeastern North Carolina and is the seat of Cumberland County. Raleigh, the state’s capital, is approximately 65 miles north of Fayetteville, and Charlotte, a major commercial center, is about 200 miles west of the city. Fayetteville is located adjacent to Interstate Highway 95, a major north-south corridor that links the city to Washington D.C., Baltimore, and New York to the north and to Charleston, Orlando, and Miami to the south. Interstate 295 – the Fayetteville Outer Loop - is currently being built around the western side of the city, which will further increase Fort Bragg and the city’s direct access to Interstate 95. Fayetteville has a plethora of historic sites, museums, local breweries, local sporting events, and shopping areas across the city
