Hartz Search is excited to announce the placement of Dionne Leverette as the Director of Clinical Care Management at Southeast Georgia Health System!
After an extensive national search, Southeast Georgia Health System selected Dionne Leverette as the Director of Clinical Care Management, who will be responsible for providing oversight and broad direction to case management, including utilization review, social services, and discharge planning for the health system. The Director of Clinical Care Management will work closely with clinical staff and medical staff to ensure that patients are classified in the appropriate level of care, monitored during their hospitalization, and have an appropriate length of stay with a discharge plan to ensure safe, high-quality outcomes.

Dionne joins Southeast Georgia Health System after working as the Interim Director of Case Management Services for Tenet Saint Vincent’s Hospital. Prior to that, she worked at the City of Hope Comprehensive Cancer Center as the Executive Director for Case Management and Patient Throughput. Through her leadership, Dionne ensures patients have access to and benefit from quality care coordination services to ensure a safe transition of care, with the understanding that good care management and appropriate LOS management reduce costs, reduce patient harm, increase patient safety, and provide a better patient experience.
Dionne holds a Bachelor of Science in Nursing from the University of Virginia and a Master of Science in Health Administration/Informatics from the University of Phoenix.
