After an extensive national search, 4C selected Delano Miller as their new Executive Director. Delano will be responsible for the operations and management of Carolina Collaborative Community Care (4C) which includes clinical, financial, and human resources.

Dr. Miller joins 4C with over 20 years of healthcare and nonprofit leadership experience. Dr. Miller is committed to helping others while staying focused on the mission of the organization he serves. His greatest contribution as a leader is to develop and empower those individuals and teams around him so that they can achieve professional and personal growth. Dr. Delano also serves as a Pastor of his church and is serving in an advisory role with a non-profit Christian Education Organization.
Carolina Collaborative Community Care is an NCQA accredited organization incorporated in 2004 that offers care management and disease management services in Cumberland and surrounding counties. 4C maintains a focus on population management, recognizing and addressing medical and non-medical social drivers of health and well-being to ultimately improve the quality of patients’ lives.
